Fishin' Galore Volunteer Application 2026

THIS IS AN APPLICATION TO VOLUNTEER AT FISHIN' GALORE.
IF YOU ARE APPROVED TO ATTEND FISHIN' GALORE, YOU WILL RECEIVE AN EMAIL AT A LATER DATE THAT WILL INCLUDE A QR CODE. YOU WILL BE REQUIRED TO HAVE THE QR CODE TO ENTER THE PROPERTY ON THE DAY OF FISHIN' GALORE.
EVERY VOLUNTEER MUST HAVE THEIR OWN UNIQUE QR CODE.
VOLUNTEERS WILL ONLY RECEIVE A QR CODE BY COMPLETING THIS APPLICATION PROCESS.

A parent or legal guardian must complete this application if the volunteer is under the age of 18.
All volunteers must be at least 14 years old.
Each Volunteer MUST apply separately.
If multiple family members are applying to volunteer, please submit an application for each person individually.
Each volunteer application will be reviewed for approval. You will be contacted when your application is approved.

Engagement

Workplace Giving is an easy way to make tax-deductible donations to Dream Day Foundation taken directly out of your paycheck. Typically Dream Day Foundation would discuss this opportunity with your Human Resources or Management to coordinate logistics, make a presentation to all employees and seek pledges from each individual to be fulfilled through paycheck deductions over a one year period. The pledges could be renewed yearly.

Volunteer of the Year Nomination

The Baton Rouge Coca Cola Bottling Company has been a dedicated partner at all Dream Day Foundation events for many years. In 2012, the young and vibrant CEO, Darian Chustz, passed away. We recognized Coca Cola as an outstanding sponsor at the 2012 Fishin' Galore in his memory. In 2013, we began to award the Darian Chustz Volunteer of the Year award to a person or group in order to recognize their outstanding commitment and support of Dream Day Foundation. The award is given out for outstanding volunteerism. Somebody who gives of themselves without hesitation. Somebody that prepares for all of the ‘just in case’ scenarios and is ready to fill in the gaps to make sure this day goes off without a hitch.

Mr. Chustz has been described as an unselfish individual who gave his time and efforts to local charitable causes and fundraisers while never directing any spotlight on himself. We would like to ask you to nominate an outstanding volunteer from the Dream Day Foundation ranks and tell us why you think this person is the most outstanding volunteer.

Please refrain from nominating any of the Dream Day Board members as they are not eligible.

Lifetime of Dedication Nomination

Dream Day Foundation was established in 1996 after St. Jude patient, Danny Bozeman, took that first fishing trip in 1994. There have been many dedicated individuals, groups, families and sponsors who have been a part of Fishin' Galore and Dream Day Foundation from the beginning. They have dedicated innumerable hours of their time to ensure the St. Jude families have the best experience year after year. This individual, group, family or sponsor may also give their time and resources before and after Fishin' Galore and/or at other events.

Photo Waiver

Effective as of the date shown below, approval for past use and permission for present and future use is being granted to Dream Day Foundation, 1165 S Foster Drive, Baton Rouge, LA 70806 to use photographs or other images taken from May 1, 2026 - May 3, 2026 of the Photographed Party, as more fully explained in this Consent and Release. The Photographed Party is an individual adult, parent or guardian and is fully authorized to sign this Consent and Release. For value received, receipt of which is hereby acknowledged, the Photographed Party hereby grants consent to Dream Day Foundation, its agents, employees, licensees, and successors in interest (collectively, the Released Party) and authorize the use of any and all photographs taken of me, and any reproduction of them in any form in any media whatsoever and in any derivative work based thereon throughout the world, and to use them to publicize, promote and advertise, including but not limited to use for point of sale advertising. The Photographed Party also consents to the use of my own name or any fictitious name which may be chosen in connection with the aforesaid photographs. The Photographed Party hereby releases any and all claims whatsoever in connection with the use of my photograph and name and the reproduction thereof as aforesaid. The Photographed Party hereby waives any right that I may have to inspect and/or approve the Book or the advertising copy that may be used in connection therewith or the use to which it may be applied. THE PHOTOGRAPHED PARTY WARRANTS THAT YOU ARE THE UNDERSIGNED AND THAT YOU HAVE READ THIS CONSENT AND RELEASE PRIOR TO THE SIGNING OF THIS DOCUMENT, THAT THE UNDERSIGNED UNDERSTANDS IT, AND THAT THE UNDERSIGNED FREELY ENTERS INTO THIS CONSENT AND RELEASE.

Yes, I do hereby hold and save harmless, Dream Day Foundation, its officers, directors, employees, and agents, from any and all liability of any nature or kind from any claim, demand, action, suit and otherwise of any type whatsoever, directly or indirectly including cost or expenses for or on account of any and all damages or injuries which may be sustained by myself and/or property resulting from my association with Dream Day Foundation, including, but not limited to, riding, working, or being on the grounds and property of the Three Bar E Ranch Fishin’ Galore and taking any action at the request of Dream Day Foundation its officers, directors, employees and agents.
