Volunteer Application

Volunteer Applications are accepted on an ongoing basis for volunteers comfortable attending our events. If you have questions, please contact your Team Lead or the Dream Day Foundation office at 225.754.5700 or info@dreamdayfoundation.org.

Completing the Volunteer Application below is the first step in becoming a volunteer with Dream Day Foundation.

This form is not a registration for any event, including Fishin’ Galore.

A separate registration form will be required for each event for which you’d like to volunteer. Registration forms are published throughout the year and can be found under the Events tab.

A parent or legal guardian must complete this application if the volunteer is under the age of 18.
All volunteers must be at least 14 years old.
Each Volunteer MUST apply separately.
If mutliple family members are applying to volunteer, please submit an application for each person individually.
Each volunteer application will be reviewed for approval. You will be contacted when your application is approved.

Here's why we are asking:
Contact Information - We want to make sure we have the most up-to-date contact information so you can be sure to receive notifications about Fishin' Galore registation/announcements and other Dream Day Foundation events. We must also be able to identify you in relation to the Waiver/Consent.
Engagement - Our foundation has evolved and grown over the past 30 years and in doing so we've upgraded our system (a few times). We encourage you to let us know about your past Fishin' Galore experiences. Also, we have several speaking opportunities throughout the year including St. Jude Dream Home Giveaways, fundraising events and presentations made to groups who are encouraged to donate to Dream Day Foundation. If you are willing, we'd forever be grateful to you for speaking about your St. Jude experience and/or your Fishin' Galore experience! If you are not a public speaker, maybe you'd prefer to leave a written testimonial we could reference in print material, on social media or during these presentations. Any testimonial will be referenced by first name only, unless specific permission is granted. (Example: When we go to Fishin’ Galore and all the events Dream Day has and to be a part of that Dream Day Family, it reminds us to have life. Workplace Giving is an easy way to make tax-deductible donations to Dream Day Foundation taken directly out of your paycheck. Typically Dream Day Foundation would discuss this opportunity with your Human Resources or Management to coordinate logistics, make a presentation to all employees and seek pledges from each individual to be fulfilled through paycheck deductions over a one year period. The pledges could be renewed yearly.

Photo Waiver

Effective as of the date this form is submitted, approval for past use and permission for present and future use is being granted to Dream Day Foundation, 1165 S Foster Drive, Baton Rouge, LA 70806 to use photographs or other images taken of the Photographed Party, as more fully explained in this Consent and Release. The Photographed Party is an individual adult, parent or guardian and is fully authorized to sign this Consent and Release. For value received, receipt of which is hereby acknowledged, the Photographed Party hereby grants consent to Dream Day Foundation, its agents, employees, licensees, and successors in interest (collectively, the Released Party) and authorize the use of any and all photographs taken of me, and any reproduction of them in any form in any media whatsoever and in any derivative work based thereon throughout the world, and to use them to publicize, promote and advertise, including but not limited to use for point of sale advertising. The Photographed Party also consents to the use of my own name or any fictitious name which may be chosen in connection with the aforesaid photographs. The Photographed Party hereby releases any and all claims whatsoever in connection with the use of my photograph and name and the reproduction thereof as aforesaid. The Photographed Party hereby waives any right that I may have to inspect and/or approve the Book or the advertising copy that may be used in connection therewith or the use to which it may be applied. THE PHOTOGRAPHED PARTY WARRANTS THAT YOU ARE THE UNDERSIGNED AND THAT YOU HAVE READ THIS CONSENT AND RELEASE PRIOR TO THE SIGNING OF THIS DOCUMENT, THAT THE UNDERSIGNED UNDERSTANDS IT, AND THAT THE UNDERSIGNED FREELY ENTERS INTO THIS CONSENT AND RELEASE.

Yes, I do hereby hold and save harmless, Dream Day Foundation, its officers, directors, employees, and agents, from any and all liability of any nature or kind from any claim, demand, action, suit and otherwise of any type whatsoever, directly or indirectly including cost or expenses for or on account of any and all damages or injuries which may be sustained by myself and/or property resulting from my association with Dream Day Foundation, including, but not limited to, riding, working, or being on the grounds and property of the Dream Day Foundation office at 1165 S Foster Drive, Baton Rouge, LA 70806 and/or Three Bar E Ranch Fishin’ Galore and taking any action at the request of Dream Day Foundation its officers, directors, employees and agents.
